I hereby give consent for my son/daughter to be enrolled in the Northeast Indiana Driving Academy, LLC driver education course. The course includes thirty hours of classroom instruction followed by six hours of behind-the-wheel instruction. The driver education learner permit is not valid if my child does not complete the driver education class or meet all course requirements. When the driver education permit expires, all privileges expire with it.
The school will not refund any fee, tuition, deposit, or any portion should the school be ready, willing, and able to fulfill its part of the contract. An owner, officer, instructor, agent, or employee of this school shall not state nor give the impression to a student that upon completion of the course, they will guarantee the securing of a driver’s license to operate a motor vehicle.
The following fees will be assessed:
Course fee: Total cost of the driver education program is $395. A deposit of $100 is required with this form. Balance due before driving instruction commences. Additional drives in one-hour intervals may be purchased for $40.
Obtaining a permit: Bring this completed form into the office or mail it to the address with the deposit. You will then receive a CDE form to obtain your permit.
Attendance: One class may be deferred to another session but must be made up before you start driving. After a second absence, you will need to schedule another session.
Materials: You need both paper and picture permits during drives.
